Contemporary medicine views health as the individual’s physical, mental, and social well-being. Oral health plays a crucial role in one’s well-being, as the oral cavity and its surrounding regions execute essential functions in verbal and nonverbal communication, sensing, digestion, and significantly contribute to aesthetic appearance. The multifaceted nature of the notion of oral health, as well as the patient’s needs and autonomous will result in various treatment options for the same oral state, favouring often contrasting ethical values and different aspects (...) of oral health. The objective of this article is to suggest alternative treatment strategies in dentistry with respect to the following factors: extent of rehabilitation, preserving one’s anatomical structures, aesthetic outcome, number of sessions, patient autonomy. Additionally, this article describes the suggested treatment strategies in an ethical context and determines the conditions of their employment. The suggested treatment strategies are divided in two categories, extensive treatment strategies focusing on the patient’s entire craniofacial complex, while specific treatment strategies focus on specific paramount issues. In the realm of medical ethics, the foundational principle of respecting patient autonomy holds significant importance, often emerging as a central concern in numerous ethically complex cases, as authorizing medical assistance in dying or healthy limb amputation on patient request. Even though advocates for either alternative regularly utilize prima facie principles to resolve ethical dilemmas, the interplay between these principles is often the core of the theoretical frameworks. As the ramifications of the sustainability crisis become increasingly evident, there is a (...) growing need to integrate awareness for sustainability into medical decision-making, thus reintroducing potential conflict with patient autonomy. The contention of this study is that the ethical standards established in the 20th century may not adequately address the challenges that have arisen in the 21st century. The author suggests an advanced perception of patient autonomy that prioritizes fostering patients’ knowledge, self-awareness, and sense of responsibility, going beyond a sole focus on their intrinsic values. Empowering patients could serve as a tool to align patient autonomy, beneficence, and the aim to reduce resource consumption. (shrink)

Neural network models of memory are notorious for catastrophic interference: Old items are forgotten as new items are memorized (French, 1999; McCloskey & Cohen, 1989). While working memory (WM) in human adults shows severe capacity limitations, these capacity limitations do not reflect neural network style catastrophic interference. However, our ability to quickly apprehend the numerosity of small sets of objects (i.e., subitizing) does show catastrophic capacity limitations, and this subitizing capacity and WM might reflect a common capacity. Accordingly, computational investigations (...) (Knops, Piazza, Sengupta, Eger & Melcher, 2014; Sengupta, Surampudi & Melcher, 2014) suggest that mutual inhibition among neurons can explain both kinds of capacity limitations as well as why our ability to estimate the numerosity of larger sets is limited according to a Weber ratio signature. Based on simulations with a saliency map‐like network and mathematical proofs, we provide three results. First, mutual inhibition among neurons leads to catastrophic interference when items are presented simultaneously. The network can remember a limited number of items, but when more items are presented, the network forgets all of them. Second, if memory items are presented sequentially rather than simultaneously, the network remembers the most recent items rather than forgetting all of them. Hence, the tendency in WM tasks to sequentially attend even to simultaneously presented items might not only reflect attentional limitations, but also an adaptive strategy to avoid catastrophic interference. Third, the mean activation level in the network can be used to estimate the number of items in small sets, but it does not accurately reflect the number of items in larger sets. Rather, we suggest that the Weber ratio signature of large number discrimination emerges naturally from the interaction between the limited precision of a numeric estimation system and a multiplicative gain control mechanism. Unfounded—conspiracy and health—beliefs about COVID-19 have accompanied the pandemic worldwide. Here, we examined cross-nationally the structure and correlates of these beliefs with an 8-item scale, using a multigroup confirmatory factor analysis. We obtained a two-factor model of unfounded (conspiracy and health) beliefs with good internal structure (average CFI = 0.98, RMSEA = 0.05, SRMR = 0.04), but a high correlation between the two factors (average latent factor correlation = 0.57). This model was replicable across 50 countries (total N = 13,579), (...) as evidenced by metric invariance between countries (CFI = 0.96, RMSEA = 0.06, SRMS = 0.07) as well as scalar invariance across genders (CFI = 0.98, RMSEA = 0.04, SRMS = 0.03) and educational levels (CFI = 0.98, RMSEA = 0.04, SRMS = 0.03). Also, lower levels of education, more fear of COVID-19, and more cynicism were weakly associated with stronger conspiracy and health beliefs. In this first part of a two-part paper, we describe efforts in the early decades of this century to restrict the extent of violations of the Second Law of thermodynamics that were brought to light by the rise of the kinetic theory and the identification of fluctuation phenomena. We show how these efforts mutated into Szilard’s proposal that Maxwell’s Demon is exorcised by proper attention to the entropy costs associated with the Demon’s memory and information acquisition. In the second part (...) we will argue that the information theoretic exorcisms of the Demon provide largely illusory benefits. According to the case, they either return a presupposition that can be had without information theoretic consideration or they postulate a broader connection between information and entropy than can be sustained. In this second part of our two-part paper we review and analyse attempts since 1950 to use information theoretic notions to exorcise Maxwell’s Demon. We argue through a simple dilemma that these attempted exorcisms are ineffective, whether they follow Szilard in seeking a compensating entropy cost in information acquisition or Landauer in seeking that cost in memory erasure. In so far as the Demon is a thermodynamic system already governed by the Second Law, no further supposition about information and entropy (...) is needed to save the Second Law. In so far as the Demon fails to be such a system, no supposition about the entropy cost of information acquisition and processing can save the Second Law from the Demon. (shrink)

The Fourth Industrial Revolution means the digital transformation of production systems. Cyber-physical systems allow for the horizontal and vertical integration of these production systems as well as the exploitation of the benefits via optimization tools. This article reviews the impact of Industry 4.0 solutions concerning optimization tasks and optimization algorithms, in addition to the identification of the new R&D directions driven by new application options. The basic organizing principle of this overview of the literature is to explore the requirements of (...) optimization tasks, which are needed to perform horizontal and vertical integration. This systematic review presents content from 900 articles on Industry 4.0 and optimization as well as 388 articles on Industry 4.0 and scheduling. In Leo Szilard’s 1929 thought experiment, a Maxwell demon manipulates a one-molecule gas to reverse the second law of thermodynamics. The demon must fail, Szilard argued, since there is hidden entropy creation in the demon’s collecting of information. This thought experiment is an inconsistent muddle of improper idealizations. It diverted an already successful literature of exorcism into degenerating speculations about about a connection between thermodynamic entropy and information. These confusions persist today in a voluminous literature. Narrative conventions in a thought (...) experiment allow thought experimenters great latitude in deciding which processes are typical and bear generalization and which can be idealized away as incidental. For its abuse of this latitude and the extent of the harm it caused, this thought experiment merits the distinction of the worst thought experiment in science. (shrink)

In a recent contribution to this journal, D. Kovacs addresses the following passage from the fourth Eclogue :o mihi tum longae maneat pars ultima uitae,spiritus et quantum sat erit tua dicere facta!Kovacs takes it for granted that the meaning of l. 54 should correspond to the Loeb translation, ‘and inspiration enough to hymn your deeds!’. Starting from this assumption, he rejects the reading spiritus, arguing that a genitive is required ; the possible solution he suggests is pectoris, used metaphorically in (...) the sense of ‘poetical ability’. That seems a clever conjecture, restoring excellent style. Nevertheless, we have to assume that spiritus replaced pectoris as a gloss, but I cannot see any reason why somebody would have wanted to explain obscurum per obscurius: other words would have been far more appropriate. On the other hand, Virgil most likely chose the word spiritus, whose basic meaning is not ‘inspiration’ but ‘breath’, in order to amplify line 53. (shrink)

This paper argues that the concept and role of ‘institutional racism’ in contemporary discussions of race should be reconsidered. It starts by distinguishing between ‘intrinsic institutional racism’, which holds that institutions are racist in virtue of their constitutive features, and ‘extrinsic institutional racism’, which holds that institutions are racist in virtue of their negative effects. It accepts intrinsic institutional racism, but argues that a ‘disparate impact’ conception of extrinsic conception faces a number of objections, the most serious being that it (...) has no plausible account of what it is that makes extrinsically racist institutions racist. It also argues that claims about the explanatory indispensability of institutional racism are overstated (individual racism is at least as important), critiques structural approaches to racial inequality, and suggests that there is reason to doubt whether institutional reform can provide us with all that morality may require in the racial domain. Objective—To determine the prevalence of, and attitudes towards, observed and personal research misconduct among newly appointed medical consultants. Design—Questionnaire study.Setting—Mersey region, United Kingdom.Participants—Medical consultants appointed between Jan 1995 and Jan 2000 in seven different hospital trusts (from lists provided by each hospital's personnel department). Main outcome measures—Reported observed misconduct, reported past personal misconduct and reported possible future misconduct.Results—One hundred and ninety-four replies were received (a response rate of 63.6%); 55.7% of respondents had observed some form of research misconduct; 5.7% of (...) respondents admitted to past personal misconduct; 18% of respondents were either willing to commit or unsure about possible future research misconduct. Only 17% of the respondents reported having received any training in research ethics. Anaesthetists reported a lower incidence of observed research misconduct (33.3%) than the rest of the respondents (61.5%) (p<0.05). Conclusion—There is a higher prevalence of observed and possible future misconduct among newly appointed consultants in the UK than in the comparable study of biomedical trainees in California. Although there is a need for more extensive studies, this survey suggests that there is a real and potential problem of research misconduct in the UK. (shrink)

It was proposed that the data from Monte Carlo statistical investigations be subjected to analysis of variance methods rather than the conventional techniques of tabling, graphing, and inspecting the data. Two examples in which analysis of variance methods were applied to published Monte Carlo studies were presented. It was suggested that balanced factorial designs should be used whenever possible in Monte Carlo studies so that analysis of variance methods would be directly applicable. Finally, three advantages of analysis of variance methods (...) over conventional techniques were described and analysis of variance methods were contrasted with a possible alternative analytical method, multiple regression analysis. This is Part 1 of a paper on fibred semantics and combination of logics. It aims to present a methodology for combining arbitrary logical systems L i , i ∈ I, to form a new system L I . The methodology `fibres' the semantics K i of L i into a semantics for L I , and `weaves' the proof theory (axiomatics) of L i into a proof system of L I . There are various ways of doing this, we (...) distinguish by different names such as `fibring', `dovetailing' etc, yielding different systems, denoted by L F I , L D I etc. Once the logics are `weaved', further `interaction' axioms can be geometrically motivated and added, and then systematically studied. The methodology is general and is applied to modal and intuitionistic logics as well as to general algebraic logics. We obtain general results on bulk, in the sense that we develop standard combining techniques and refinements which can be applied to any family of initial logics to obtain further combined logics. The main results of this paper is a construction for combining arbitrary, (possibly not normal) modal or intermediate logics, each complete for a class of (not necessarily frame) Kripke models. We show transfer of recursive axiomatisability, decidability and finite model property. Some results on combining logics (normal modal extensions of K) have recently been introduced by Kracht and Wolter, Goranko and Passy and by Fine and Schurz as well as a multitude of special combined systems existing in the literature of the past 20-30 years. Purpose. The research is devoted to the analysis of the urgent problem of the information society: the overload of a person with information and, as a result, the impossibility of adequate formation and development of the personality; as well as the problem of "digitization" of human existence and the formation of a new reality of dataism. Theoretical basis. A lot of modern scientific works are devoted to the analysis of the information society, its problems and features. The information society is (...) a logical continuation of the scientific and technological revolution, which led to the rapid growth of scientific knowledge and the technology development. In the 20th century, technologies have touched the sphere of knowledge and information, as a result of which the formation and development of the information society, or "knowledge society", takes place. Information becomes the main resource in it, and, one way or another, a person’s life is inextricably connected with the information space. With the information society formation, the problem of information search and processing becomes one of the most urgent. It turns out that despite the abundance and availability of information, it is very difficult to determine its relevance. A lot of effort is spent on developing information retrieval algorithms. Another problem is related to the person’s inability to process large amounts of data. This situation begins to influence not only the education process and professional activity, but also the formation of a person’s personality. A person is "lost" in the information space and gradually loses his/her "I". Algorithms for data analysis come to the rescue, but gradually, instead of giving a person material for thought, they begin to make decisions on their own, and therefore, live life instead of a person. With the advent of Big Data processing algorithms, a new ideological paradigm appears – dataism, which predicts the merger of a person with the general data flow. Originality. The authors make assumptions that the dataistic future is "natural". A lot of works, including in the framework of philosophy, are focused on the problem of "dissolving" a person in the information space and finding ways to overcome it. But, in our opinion, this process is a completely logical continuation of human evolution. Conclusions. Existence of a person as a data flow is not a problem and a threat, but a new dimension of his/her being, and, therefore, requires a careful study and formulation of the main principles of this form of existence.
